Field-Based Community Health Advocate
LSMA Management, Inc.
LSMA Management, Inc. is seeking a Community Health Worker to support Enhanced Care Management (ECM) members by improving access to care, promoting whole-person health, and addressing social and behavioral barriers. This field-based role conducts outreach, engagement, and coordination with CB-CME, providers, and care teams.
