As a Home Health RN, you will play a critical role in providing high-quality, patient-centered care to individuals in their homes, promoting their independence and well-being. Key Responsibilities: Conduct comprehensive assessments of patients' health status and develop individualized care plans. Administer medications, treatments, and procedures as prescribed by the physician. Monitor patients' progress and adjust care plans as necessary to ensure optimal health outcomes. Educate patients and their families on managing illnesses or injuries and promote healthy lifestyle choices. Collaborate with other healthcare professionals to coordinate patient care and ensure continuity of services. Maintain accurate and timely documentation of patient care activities and progress notes. Ensure compliance with all state and federal regulations and standards of care. Qualifications: Current and unrestricted RN license in the state of Minnesota. Bachelor's degree in Nursing (BSN) preferred. Minimum of one year of nursing experience. Require Home Health Experience. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Reliable transportation and valid driver's license.
