Registered Nurse - Community Outreach

Job Summary Registered Nurse – PRN. The RN is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient's hospital stay and coordinates planning with other disciplines using a patient/customer driven approach. The position is located at MO-SSM Health Saint Louis University Hospital, 1201 Grand. Key Responsibilities Perform comprehensive nursing assessment/reassessment. Adopt age‑appropriate admission or transfer assessment and obtain input from family/guardian when appropriate. Accurately and completely document findings. Prescribe assessment for post‑op/post‑invasive procedure patients. Assess education and discharge needs of patient and family on admission and throughout hospitalization. Document patient reassessments, pertinent observations, and responses to care etc. following the patient plan of care. Establish, coordinate, and evaluate a plan of care based on assessment data, patient diagnosis, laboratory results, physician orders, protocols, and other relevant information. Identify short‑ and long‑term goals based on patient care needs. Formulate nursing interventions to achieve desired outcomes. Incorporate disease‑specific evidence‑based practice into the nursing care plan and other documentation. Provide and document nursing interventions based on assessed patient needs, plan of care, and changes in patient status. Collaborate with appropriate health team members to coordinate the daily plan of care for assigned patients. Provide, coordinate, and communicate patient care, including accurate handoff communication reports. Administer and document medications accurately according to policies. Monitor, maintain, and document accurate IV fluids and parenteral nutrition as per policies. Complete referrals as indicated by assessment data. Request consultation for special needs, equipment, or information for patient and/or family. Provide patient/family education and discharge planning per documentation guidelines and protocol. Clarify all physician orders as warranted and acknowledge and implement them accurately and timely. Assist physician with procedures/treatments as requested or delegate to a care partner when appropriate. Document readback for all telephone or verbal orders. Take telephone or verbal orders only in emergency situations. Recognize changes in patient condition and take appropriate nursing actions. Involve the family/guardian in care and decision‑making as appropriate. Recognize risks for patient and take appropriate action. Incorporate use of infection control practices in daily care. Document and or communicate nursing care and/or changes in patient condition. Perform and document ongoing evaluation of the effectiveness of care based on assessment data, interventions, and patient response. Evaluate the effectiveness of patient/family education. Modify the plan of care as indicated. Recognize significant changes in patient clinical parameters and report immediately. Identify problems, gather relevant data, suggest solutions, communicate appropriately, and work toward problem resolution. Report variations from care/treatment as per reporting policy. Provide specialized care to patients at high risk for injury. Restraint Care – Initiate and evaluate alternatives to restraint prior to application; apply restraints consistently with approved procedures; monitor and assess patient’s response; provide specified patient care; consult peers; document thorough use and associated care. Pain Management – Assess patient for pain; incorporate cultural/spiritual beliefs; implement prevention‑focused techniques; involve patient/family; consider alternative methods such as massage, repositioning, immobilization, and music therapy. Abuse Assessment – Recognize abuse criteria; report signs of abuse/neglect promptly; take appropriate action to support patient safety. Demonstrate accountability for own professional practice by adhering to quality standards, participating in learning experiences, maintaining current knowledge, attending meetings, and agreeing to continuous improvement activities. Build professional relationships with coworkers, maintain a collaborative team approach, serve as a resource person, and delegate responsibilities appropriately. Follow safety and infection control policies, practice standard precautions, maintain a safe environment, and complete all mandatory training and competency requirements. Provide service excellence to patients and families through respectful, courteous, and timely care, effective communication, and privacy protection. Incorporate performance improvement and evidence‑based practice into individual professional practice through unit goal development, data collection, solution implementation, and participation in research and improvement projects. Provide age‑appropriate care for adult or geriatric patients as required. Use resources efficiently, document all cost‑effective practices, and prioritize responsibilities to meet patient needs promptly. Job Requirements & Specifications Licensure: Registered Nurse licensed to practice in the state of Missouri. Experience: Experience in medical‑surgical nursing preferred but not mandatory for PRN shifts. Physical Requirements: Occasional prolonged standing, walking, and lifting; ability to reach, stoop, bend, kneel, or crouch as needed. Working Conditions: Exposure to blood‑borne diseases; work in unpredictable and emergency situations; irregular hours with the possibility of overnight or weekend shifts. Shift: PRN / Per Diem.
