Advanced Practice Provider (APP)
River Valley Family Health Centers is looking for an experienced Advance Practice Provider (APP) to join their clinics in either Montrose or Delta Colorado. The Advanced Practice Provider plays a critical role in delivering high-quality, patient-centered care within a community health setting. This position encompasses both clinical expertise and a commitment to improving access to healthcare for underserved populations.

Practice Highlights: River Valley Family Health Centers (RVFHC) mission is to provide access to quality medical health care in a comprehensive and culturally respectful manner to individuals and families. RVFHC is a community health center providing integrated physical, behavioral and dental health to over 9,000 individuals in Montrose and Delta Counites at our 3 clinic sites. As a NCQA Primary Care Medical Home (PCMH), RVFHC is dedicated to improving the quality of our patient's lives every day by focusing on the quadruple aim: improving population health, controlling total healthcare costs, improving the patient experience, and being an employer of choice.
Community Highlights: Situated between Delta and Montrose on the Uncompahgre River, Olathe is known for its hardworking farmers and hometown hospitality. Much of the area surrounding Olathe is public land that makes for incredible adventures — mountain biking trails, off road havens, campsites under star-filled skies and peaks begging to be hiked.
Other: This position is full time, salaried exempt, and pays $118,779.00 - $136,011.20 per year, depending on experience and certifications.
Qualifications: Unrestricted and current PA-C Certification with NCCPA, or current FNP license, in the State of Colorado, and DEA registration. Ability and willingness to treat and prescribe for Medication for Opioid use Disorder (MOUD). Ability and willingness to treat children of all ages, including infants, and performing well-child exams preferred. Current BLS/CPR and ACLS certification.
