• Conduct evaluations for patients in both inpatient and outpatient environments, crafting tailored treatment plans to meet their unique needs.
• Implement physical therapy interventions concentrated on improving mobility, strength, balance, and overall function.
• Collaborate with a diverse team of healthcare professionals, including nurses, physicians, and case managers, to enhance patient outcomes and experiences.
• Educate both patients and their families on therapeutic goals, preparing them for discharge, and guiding them on at-home exercises.
• Keep thorough documentation of evaluations, treatments, and patient progress in line with facility protocols and regulatory requirements.
• Participate in interdisciplinary rounds and team meetings, ensuring seamless communication and coordinated care.

Qualifications

For this role, your educational background should include:

• A Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution.

Certifications needed:

• State licensure as a Physical Therapist.
• CPR certification is preferred.
• Specialized certifications (e.g., neurological, geriatric) are a valuable addition but not mandatory.

Essential skills include:

• Previous experience in a therapeutic setting is highly valued.
• Ability to manage patients with a range of complex medical and mobility challenges.
• Strong communication and teamwork abilities.
• Comfort with digital documentation and clinical charting systems.
