Part-Time/PRN Physical Therapist (PT)

We are seeking a part-time/prn physical therapist (pt) to join our home health team in chicago, il area. one home health: overview one home health is a full-service, medicare-certified home health agency licensed in illinois and indiana. our mission is to deliver compassionate, personalized in-home care through a highly trained, world-class team committed to exceptional clinical outcomes and patient satisfaction.

what you'll do

perform in-home physical therapy evaluations and treatments
develop and implement individualized rehabilitation plans of care
monitor patient progress and adjust treatment plans as needed
educate patients and caregivers on exercises, mobility, and safety
document visits accurately and efficiently in accordance with agency guidelines
communicate patient progress with the interdisciplinary care team
ensure compliance with clinical, documentation, and regulatory standards

what we're looking for

licensed physical therapist (pt)
doctorate in physical therapy (dpt)
home health experience preferred, but not required
strong communication and documentation skills
ability to work independently in a home-based setting
professional, dependable, and patient-focused approach
physical ability to perform in-home visits
