Part-Time/PRN Physical Therapist (PT)
We are seeking a part-time/prn physical therapist (pt) to join our home health team in chicago, il area. one home health: overview one home health is a full-service, medicare-certified home health agency licensed in illinois and indiana. our mission is to deliver compassionate, personalized in-home care through a highly trained, world-class team committed to exceptional clinical outcomes and patient satisfaction.

what you'll do
- perform in-home physical therapy evaluations and treatments
- develop and implement individualized rehabilitation plans of care
- monitor patient progress and adjust treatment plans as needed
- educate patients and caregivers on exercises, mobility, and safety
- document visits accurately and efficiently in accordance with agency guidelines
- communicate patient progress with the interdisciplinary care team
- ensure compliance with clinical, documentation, and regulatory standards
what we're looking for
- licensed physical therapist (pt)
- doctorate in physical therapy (dpt)
- home health experience preferred, but not required
- strong communication and documentation skills
- ability to work independently in a home-based setting
- professional, dependable, and patient-focused approach
- physical ability to perform in-home visits
