CASE MANAGER

Altru Rehabilitation Hospital seeks a compassionate Case Manager to coordinate care for inpatient and outpatient rehab patients in our Healthcare & Medical Services department. The Case Manager will manage caseloads from admission through discharge, complete assessments, develop individualized care plans, facilitate interdisciplinary team collaboration, and secure post-acute services and community resources to support long-term recovery.
