Develop and implement individualized treatment plans targeting communication and swallowing disorders. Provide direct therapy, document progress notes, and update the interdisciplinary team on patient status. Collaborate with nursing, therapy, and medical staff to coordinate resident care and discharge planning. Monitor progress, adjust treatment plans as needed, and participate in care conferences. Ensure timely and accurate documentation and compliance with all applicable policies and regulations.
