About the Role:

Anuco Home Health Care LLC is looking for a compassionate and organized Intake Coordinator to join our growing team in Chicago, IL. In this vital role, you'll be the first point of contact for patients and families seeking home health care services, helping connect them with the care they need. Responsibilities:

Manage the intake process for new patients, including gathering demographic, insurance, and clinical information

Coordinate with referral sources such as hospitals, physicians, and discharge planners to facilitate smooth patient transitions

Verify insurance eligibility and obtain authorizations for home health care services

Communicate with patients, families, and caregivers to explain services and set expectations

Maintain accurate and up-to-date patient records in the electronic health record (EHR) system

Collaborate with clinical and scheduling teams to ensure timely start of care

Follow up on pending referrals and track intake metrics to support agency growth

Requirements:

Prior experience in home health care, medical intake, or healthcare administration preferred

Knowledge of home health insurance verification and prior authorization processes

Strong communication and interpersonal skills with a patient-centered approach

Proficiency with EHR/EMR systems and Microsoft Office Suite

Ability to multitask and prioritize in a high-volume, deadline-driven environment

Detail-oriented with strong organizational and data entry skills

High school diploma or equivalent required; associate's or bachelor's degree in a related field a plus
