What You'll Do
As a Clinical Coordinator, you will:
Maintain up-to-date knowledge of RHD programs and ensure implementation of best practices. Remain knowledgeable about program regulations, community support services, licensing requirements, and funding sources. Build and strengthen relationships with program staff, healthcare providers, and community partners to coordinate care and services. Maintain tracking systems to identify individuals requiring comprehensive treatment planning. Conduct monthly and quarterly coordination visits with internal and external providers. Provide direct supervision and support to Care Coordinators and Resource Coordinators. Facilitate routine multidisciplinary treatment team meetings and collaborative planning sessions. Monitor and report quality outcomes, statistics, and performance measures. Complete weekly and monthly reporting requirements accurately and timely. Conduct systematic reviews of client records to ensure comprehensive care coordination and resource utilization. Develop comprehensive treatment plans for individuals receiving services across multiple programs and settings. Review treatment plans to ensure compliance with program, funding, and regulatory requirements. Collaborate with internal departments and external partners to promote quality care and positive outcomes.

Qualifications
Required
High School Diploma or equivalent with three (3) years of relevant work experience Two (2) years of experience working with children and/or adults with Mental Health diagnoses, Intellectual and Developmental Disabilities (IDD), and/or Drug & Alcohol treatment populations Valid Driver's License Strong organizational, communication, and interpersonal skills Ability to collaborate effectively with multidisciplinary teams
Preferred
Bachelor's Degree in Human Services or a related field Experience with treatment planning, care coordination, and case management Supervisory or leadership experience within human services or behavioral health settings
Schedule
Full-Time Philadelphia & Montgomery Counties, PA Schedule may vary based on program and business needs
Physical Requirements (If Applicable)
Ability to travel between program locations and provider sites as needed Ability to sit, stand, walk, and perform administrative duties throughout the workday Ability to perform essential job functions with or without reasonable accommodation
