Licensed Practical Nurse – Home Health

Schedule: 8hrs

Assignment Details: Guaranteed Hours: 40 Contract Weeks: 12

Requirements

• Current Nursing License in the Practicing State
• Valid Driver's License
• Current CPR Certification
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Ability to use electronic medical charting and time-keeping software
• Reliable transportation and current auto liability insurance coverage
• Must read, write and speak fluent English

Responsibilities

• Administer skilled nursing care to patients in their place of residence, as prescribed by the physician.
• Assist in providing nursing services and identifying the patient's physical, psycho-social, and environmental needs.
• Participate in planning, coordinating, and implementing care in conjunction with the Registered Nurse.
• Provide effective communication to patients, families, and healthcare professionals.
• Assist patients and families in learning appropriate self-care techniques.
• Adhere to infection control measures to ensure safety for both staff and patients.
• Assist with admitting, discharging, and transferring of patients.
