General Purpose The primary purpose of this position is to provide direct nursing care to residents and supervise daily nursing activities performed by nursing assistants, in accordance with federal, state, and local standards, guidelines, and regulations. The supervisor will ensure the highest degree of quality care at all times. Essential Duties These tasks represent the core responsibilities of the role. Duties may extend beyond this list if necessary. Administrative Functions Direct the day‑to‑day functions of nursing assistants in accordance with rules, regulations, and guidelines. Ensure nursing personnel comply with written policies and procedures. Periodically review department policies, procedure manuals, and job descriptions; make recommendations for revisions. Plan shifts’ services, programs, and activities with nursing staff and support personnel. Maintain the Nursing Service Procedures Manual as current and accurate. Prepare and submit written and oral reports and recommendations regarding shift activities. Coordinate nursing services with other resident services to maintain total resident care. Ensure compliance with job descriptions and nursing service procedures. Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing service department. Assist with facility surveys and inspections by authorized government agencies. Review and update residents’ written discharge plans as required. Assist in planning the nursing portion of resident discharge plans. Interpret department policies and procedures for personnel, residents, visitors, and government agencies. Admit, transfer, and discharge residents as needed. Complete accident and incident reports. Write resident charge slips and forward to the Business Office. Maintain the Daily Census Report and submit to the Business Office. Perform other administrative duties such as completing medical forms, reports, evaluations, studies, and charting. Maintain confidentiality of user ID code and password for resident/facility information. Report violations of confidentiality or unauthorized information system access. Charting and Documentation Complete and file required record‑keeping forms and charts upon admission, transfer, or discharge. Encourage physicians to review treatment plans and record their orders and progress notes. Record physician telephone orders on the Physicians’ Order Form. Transcribe physician orders to resident charts, medication cards, and treatment plans. Chart nurses’ notes accurately, reflecting care provided and resident response. Submit accident or incident reports to the Director as required. Record new or changed diet orders and forward information to Food Services. Document discrepancies in physician orders, diet changes, or charting errors and report to Nurse Supervisor. Complete transfer forms according to established procedures. Document unauthorized discharges appropriately in the medical record. Perform routine charting duties in line with charting policies and procedures. Sign and date all entries in the resident’s medical record. Drug Administration Functions Prepare and administer medications as ordered by the physician. Verify resident identity before medication administration. Ensure prescribed medication for one resident is not given to another. Maintain adequate floor stock of medications, supplies, and equipment; report needs to Nurse Supervisor. Order prescribed medications, supplies, and equipment as needed. Maintain accuracy of narcotic records for the shift. Report drug and narcotic discrepancies to Nurse Supervisor. Review medication cards for completeness, transcription accuracy, and stop‑order adherence. Notify the attending physician of automatic stop orders prior to last dosage. Dispose of drugs and narcotics per established procedures. Personnel Functions Participate in employee performance evaluations and staffing determinations. Inform Nurse Supervisor of staffing needs when personnel fail to report. Report absentee call‑ins to Nurse Supervisor. Review and evaluate workforce, making staffing recommendations. Develop work assignments and assist staff in completing them. Provide leadership to nursing personnel on shift. Conduct daily rounds to ensure standards are met; report issues to Nurse Supervisor. Ensure nursing assistants are certified or have completed an approved training program. Ensure nurse aide trainees are supervised by a licensed nurse. Hold regular meetings with staff to identify and correct problem areas. Enforce department policies and procedures for residents and visitors. Maintain positive rapport with inter‑department personnel. Create a warm and calm work environment. Review complaints and grievances; report to Nurse Supervisor. Ensure disciplinary actions are fair and non‑discriminatory. Provide nursing report upon shift start and end. Report occupational exposures per facility policies. Report suspected fraud incidents to the Administrator. Ensure computer workstations are properly logged off or secure when unattended. Nursing Care Functions Notify staff of new admissions and room assignments. Prepare rooms for new admissions. Welcome new residents and escort them to rooms. Assist in resident/family orientation. Participate in physician rounds as needed. Request diagnostic and therapeutic services as ordered. Consult physicians on resident care, treatment, and rehabilitation. Review resident charts for treatments, medication orders, and diets. Implement and maintain nursing objectives and standards. Conduct periodic checks to ensure proper medication administration and assess resident status. Ensure care is provided by appropriately qualified personnel. Coordinate social and activity programs with nursing schedules. Notify physicians of accidents or incidents involving residents. Inform physicians and next‑of‑kin of changes in resident condition. Assist in restorative and rehabilitative programs. Inspect nursing service treatment areas for cleanliness and safety. Administer professional services (e.g., catheterization, suction, dressings, colostomy, blood draws, massages, ROM exercises, care of dying residents). Use restraints as necessary according to policy. Collect lab specimens (sputum, urine, etc.). Take and record vitals as required. Monitor seriously ill residents. Check family‑brought foods for dietary compliance. Ensure care aligns with resident care plans and wishes. Check residents who cannot call for help frequently. Meet with residents and family as needed; report issues. Admit, transfer, and discharge residents as necessary. Arrange transportation for discharged residents. Escort discharged residents to pickup area. Inform families of resident death and coordinate funeral homes as requested. Staff Development Organize and conduct in‑service training classes on job tasks. Implement an orientation program for new employees. Standardize work methods. Train staff on blood and body‑fluid exposure risks. Assist Director with planning clinical supervision for nurse aide trainees. Attend outside training and annual facility in‑service training (OSHA, TB, HIPAA, Abuse Prevention, Safety, Infection Control). Participate in advance directive and other community trainings. Engage in continuing education to maintain licensure. Safety and Sanitation Monitor staff compliance with safety regulations for equipment and supplies. Enforce organization policies, including dress codes. Assist in identifying and documenting tasks with potential blood or body‑fluid exposure. Ensure staff complete required in‑service training before performing exposure‑risk tasks. Maintain adequate personal protective equipment supply. Keep nursing station, drug rooms, and resident rooms clean and sanitary. Enforce hand hygiene protocols. Ensure staff participate in fire safety and disaster drills. Support the infection control program for monitoring communicable diseases. Adhere to isolation precautions when required. Control use and disposal of personal protective equipment. Maintain procedures for reporting hazardous conditions or equipment. Ensure safe use of lifting equipment and PPE during resident transfers. Report missing or illegible labels and MSDSs to safety officer. Equipment and Supply Functions Recommend equipment and supply needs to Nurse Supervisor. Maintain adequate stock levels of medications, supplies, and equipment. Assist in developing and implementing safe operation procedures for nursing service equipment. Ensure only trained personnel operate equipment. Monitor efficient use of supplies to prevent waste. Keep MSDSs for hazardous chemicals on file. Care Plan and Assessment Functions Review daily care plans to ensure appropriate care. Inform Nurse Supervisor of necessary changes to care plans. Document that care plans are followed in nursing notes. Review resident care plans for goals, problems, and revisions. Ensure CNAs review care plans before administering daily care. Assist in planning, scheduling, and revising MDS and RAPs. Budget and Planning Functions Report suspected fraud related to false billing, cost reports, or kickbacks.
