CARE COORDINATOR - Pueblo

Job Location: Pueblo, CO 81006. Salary Range: $20.25 per hour. Responsibilities Develop care planning and conduct patient needs assessments. Facilitate life skills groups in accordance with CTP policies and procedures. Serve as liaison and establish networks with hospitals, community health centers, state agencies, and human service agencies regarding patient services and needs. Document admissions, care management needs, discharges, and discharge planning. Administer case management assessments, ensuring completion of the Addiction Severity Index (ASI) at intake. Organize a schedule of patient bed dates, transition levels of care, and maintain consistent communication with Medicaid Placement Coordinators. Assist treatment team with patient intake, transfer, and discharge assessments. Collaborate with facility treatment team (medical, psychiatric, and SUD) to identify patients’ needs. Help patients meet basic needs such as clothing, medical care, legal support, and religious services. Qualifications Education : Associate or bachelor’s degree in a behavioral health-related field is preferred. Obtain the Certificate of Addiction Technician (CAT) within 18 months from hire date. Obtain case management certification within 90 days of hire. Experience : Minimum two years working in the substance misuse and mental health field. Other : Good understanding of substance misuse, mental health, and treatment delivery systems. Advanced computer skills required. CPR and First-Aid certification (coverage provided). Ability to take vital signs, recognize medical problems, and refer to medical staff. Valid driver’s license and good driving record. Effective communication within a multidimensional treatment program that includes medical, psychiatric, and substance use components.
