You benefit from a cohesive, in-house therapy team and supportive organization with opportunities to learn and grow.Optage is the Home & Community division of PHS and is seeking a Full Time Physical Therapist to work in the home health care and integrated care environments to serve our residents and community members throughout the Twin City Metro area.Hiring Bonus - $5,000SCHEDULE/HOURS: Monday - Friday and rotating weekendsWhy Become a Therapist with PHS?Great work/life balance and flexible schedulesCompetitive pay and annual increasesUse your strengths to serve multiple settings – TCU/SNF, Assisted Living, Independent Living, Home Health, and OutpatientSpend more time with your residents/clients with reasonable productivity expectationsPaid mileage and drive time if traveling between communitiesAccess to online continuing education courses & podcastsScholarship program – awards up to $5,250 per year!May be eligible for Public Service Loan ForgivenessOpportunities for growth and internal promotion!ResponsibilitiesAs a Physical Therapist you will be responsible to:Conduct a comprehensive assessment & evaluation using subjective and objective measures.Develop treatment plans and programs in accordance with physician orders.Provide quality treatments, actively progressing goals to achieve highest quality of life.Conduct screens and provide education to residents/clients, families, caregivers, and staff.Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.Provide accurate and timely documentation & billing in electronic medical record.Demonstrate understanding and knowledge of payer sources and regulatory requirements.Complete and document supervisory visits per regulations.Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.QualificationsGraduate from an accredited Physical Therapy program.Current licensure in the state(s) of practice as required.Demonstrated compatibility with PHS's mission and operating philosophies.Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.Pay RangeThe estimated starting pay range for this role is $40.46 - $57.12 per hour. This good faith estimate is based upon PHS's typical starting pay range for this role.
