Description

• Assess patients in inpatient or outpatient environments and develop tailored treatment plans
• Implement physical therapy interventions targeting mobility, strength, balance, and function
• Collaborate effectively with nurses, physicians, case managers, and other rehab professionals to achieve optimal results
• Instruct patients and their families on therapeutic goals, discharge planning, and home exercises
• Document evaluations, treatments, and patient progress according to facility and regulatory guidelines
• Engage in care team rounds and interdisciplinary conferences to maintain coordinated care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from a regionally accredited program

Certifications

• Current state licensure as a Physical Therapist
• Current CPR certification is preferred
