Position Summary Clinical Care Coordinator, Mental Health City of New York, New York, NY Medical, Retirement, Full‑Time Licensure Requirements Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW) as required by NYS Department of Mental Hygiene. Licensed under a limited permit valid for up to twelve months, with supervision under a licensed master or clinical social worker, as per NYS regulations. Job Description Receive and process SPOA referrals to determine appropriate level of care with the supervisor, consumer, and referral source. Ensure receipt of proper SPOA referral documentation, requesting and tracking supportive records. Assess eligibility, review routine and urgent cases, and determine appropriate level of care. Identify threshold criteria and consumer pathways for care coordination of individuals with mental illness. Review and reassess risk to inform prioritization of assignment. Collaborate with Health Home staff, community providers, referral sources, and other professionals to inform eligibility, case assessment, and assignment. Monitor caseload of high‑risk clients and ensure adequate follow‑up frequency before treatment assignment. Reassess risk, violence potential, and provide provider consultation/technical assistance as needed. Escalate situations that require higher‑level attention or intervention by other service systems. Maintain accurate records of all contacts and decision‑making processes. Keep supervisor, team members, referrer and consumer informed regarding plan of care and any referral issues. Enter current and accurate client data into the database to ensure complete client records. Complete other duties or tasks as assigned on an as‑needed basis. Desired Qualifications Master’s Degree in Social Work from an accredited school of social work. A valid Certified Social Worker (CSW) certificate or Licensed Master of Social Work license (LMSW) issued by the New York State Department of Education. Preferred Skills At least 2 years of experience in a mental health setting. Knowledge of the behavioral health service delivery system in New York City. Excellent interpersonal and communication skills with strong organizational abilities. Experience in clinical assessment, familiarity with DSM‑V, risk assessment, diagnostic categorization, and client disposition/planning. Proficiency with basic data entry.
