Adventist HealthCare Home Health is hiring a Physical Therapist (PT) in our Silver Spring territories. The PT will provide care in acute care, outpatient, or inpatient rehabilitation settings, following the organization’s mission to extend God’s care through the ministry of physical, mental and spiritual healing. Responsibilities Perform an initial examination and evaluation to establish impairment/diagnosis, prognosis and plan of care prior to intervention. Provide or direct and supervise therapy interventions consistent with the examination, evaluation, diagnosis, prognosis, and plan of care. Involve the patient/client and appropriate others in planning, implementation, and assessment of the care plan, reexamining as needed. Plan for discharge, considering goal achievement and expected outcomes, and arrange appropriate follow‑up or referral. Communicate, coordinate and document all aspects of patient management with the care team, and supervise assigned rehab aides, office assistants and volunteers. Manage schedule to accommodate census and patient load and inform management of caseload issues. Attend team meetings and effectively communicate patient status, using the medical record or other systems as needed. Be responsible for individual professional development activities. Qualifications Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program. One year of site‑specific experience preferred. Active Maryland license. Valid driver’s license with agency insurance requirements. Active American Heart Association Basic Life Support (BLS) certification.
