Job Summary
Lily of the Valley is seeking a skilled and compassionate  PRN Registered Nurse (RN)to provide high-quality recuperative care or Home Care Nursing on an as-needed basis. The PRN RN will assist patients recovering from hospital stays, ensuring they receive proper care, medication management, and support to promote a smooth transition to independent living. The RN will work under the supervision of a clinician.

Responsibilities

• Provide comprehensive nursing care, including assessments, wound care, and medication administration.
• Highly qualified; can also perform assessments.
• Ability to give injections e.g at-home B12 injections to Long-term, high-needs clients.
• Monitor patients’ progress and respond to changes in their medical conditions.
• Collaborate with physicians, case managers, and support staff to develop personalized care plans.
• Educate patients on self-care, medication adherence, and wellness strategies.
• Ensure compliance with infection control and safety protocols.
• Document all patient interactions, assessments, and interventions accurately in medical records.
• Support patients transitioning to permanent housing, rehabilitation, or other care settings.
• Administer medications and treatments as prescribed, ensuring compliance with HIPAA regulations.
• Maintain a safe and clean environment for patients by adhering to infection control standards.
• Participate in training programs to stay updated on best practices in internal medicine and long-term care.

Qualifications/Experience   :

• Active RN license in Minnesota
• Experience, preferably in recuperative care, home health, med-surg, or community health.
• Strong clinical assessment and critical thinking skills.
• Experience working with vulnerable populations, including individuals experiencing homelessness or chronic health conditions, is a plus.
• Ability to work independently while being a collaborative team player.
• Excellent communication and patient education skills.
• Flexibility to work PRN shifts, including weekends and holidays, as needed.
• A commitment to upholding patient dignity, confidentiality, and ethical standards in all interactions.
