Lead your team of CNAs in a supportive way that inspires them to do their very best!Full Time Every other weekend- 8-hour shifts- 230pm-11pmDuties/ResponsibilitiesAssist in the planning development organization implementation evaluation and direction of assigned shifts with administration medical and nursing staff and related departments in planning department services programs and in development implementation and maintenance of nursing objectives and in development implementation and coordination of departmental policies and procedures resident care plans nursing procedure manuals and job that departmental personnel residents and visitors follow established policies and procedures at all involved in the planning and implementation of recommendations by the Care Plan Team RISK Committee Utilization Review Committee and Medicare Stand Up that documentation of residents daily medical record is accurate comprehensive and document and implement physicians orders in an accurate and timely admit and discharge of residents to and from the Smith Care timely response to resident requests/call Home Care services and Smith Care Center services as the proper recording and accurate count of schedule drugs at all security of locked Smith Care Center areas; monitor that only authorized staff is in possession of Smith Care Center accurate and comprehensive reporting of resident condition via shift-to-shift departmental policies and procedures to personnel residents visitors and family each resident at least once during a shift to ensure appropriate care is being rendered and act that Residents Bill of Rights is followed at all that residents have a clean safe orderly and comfortable in the development of and participate in programs designed for in-service education. Monitor orientation for unlicensed new hires. Orient licensed new and maintain rapport with the medical profession and other health related facilities and in identifying departmental problems and subsequent recommendations to the Director of Health Services regarding staffing improvement of services and potential problem in discharge planning development and implementation of resident care plans and resident administrative duties as required by departmental policies and procedures and report to the Director of Health in coordinating transportation to other health care facilities and individuals as in departmental projects and studies as health services i.e. admissions discharges that a stock level of medications medical supplies equipment and other supplies is nursing care service areas and practices regularly to ensure compliance with current applicable with residents physician in planning nursing care; periodically reviewing and updating care plans with MDS medical forms according to departmental policies and the residents attending/alternate physician family or guardian of any change in condition accident or incident according to established facility cases of death implement arrangements according to residents families or guardians wishes and in compliance with facility emergencies determine emergency measures to be used and notify the Director of Health Services of the in the standardization of methods in which work will be RESPONSIBILITYThe LPN has supervisory responsibility for Certified Nursing of an accredited school of licensed practical one year in the field of Long-Term Care have good organizational be a detail-oriented person and able to perform duties with great accuracy on a daily have excellent personal skills demonstrating the ability to conduct oneself in a non-controversial style; a style that inspires the respect of others and promotes a sense of trust in the individuals have good communication skills; speaking writing and be fluent in have good computer skills with the ability to learn and use proprietary software as have valid certification as a Licensed Practical Nurse in the State of have a current CPR (Cardiopulmonary Resuscitation) certification.
