Hospital Clinical Liaison

Care Transition Nurse / Hospital Liaison PurposeCare is a leading provider of home care and home health services in the Midwest. We are looking for a relationship-driven and operationally strong Care Transition Nurse to support hospital partnerships and help patients transition smoothly into home health services. Responsibilities Coordinate home health referrals and patient transitions from hospitals into home-based care Partner closely with hospital case managers, discharge planners, social workers, and healthcare teams Manage and follow up on referrals with urgency and strong attention to detail Educate patients and families on home health services and available support options Conduct regular onsite hospital visits and maintain strong referral relationships Collaborate with PurposeCare Intake and Clinical Operations teams to support timely Starts of Care Utilize systems such as CarePort, WellSky, Axxess, Trella Help identify opportunities to improve referral conversion and patient experience Support awareness of both home health and private pay service offerings Qualifications LPN or RN required Minimum of 3 years of experience in home health, care transitions, clinical liaison work, hospital case management, or healthcare operations Strong relationship-building and customer service skills Ability to manage multiple priorities in a high-volume healthcare environment Strong attention to detail and follow-through Experience working with referral platforms and EMR systems preferred Strong critical thinking and communication skills Comfortable working independently in the field while collaborating closely with operational teams Benefits Medical & Prescription Drug Plan Vision 401K Life Insurance Short- & Long-Term Disability Insurance EEOC Statement All employment decisions at PurposeCare are based on business needs, job requirements, and individual qualifications.
