RN Complex Care Manager — Diabetes Care Specialist
Lucent Health Solutions LLC
Lucent Health is seeking a Complex Care Manager RN to provide onsite and telephonic case management for participants with chronic or catastrophic illnesses, aiming for high-quality, cost-effective care in line with CCM/CMSA standards. Preferred credentials include CCM/CRRN/OCN; a CDE certification is strongly preferred or an RN with diabetes management experience willing to obtain CDE.
