Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs and physician direction. Responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, the therapist ensures design of functional treatment programs allowing for effective return to community activities. Additionally, the Physical Therapist serves as an educator for patients, families, students, and coworkers.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop treatment plans focused on progressing patients towards discharge.
• Collaborate with the interdisciplinary team to address all barriers to discharge, including timely equipment trialing and ordering.
• Ensure that care is provided under signed, valid physician orders and within insurance approvals as necessary.
• Manage individual, concurrent, and group treatment sessions effectively based on patient care needs.
• Complete medical record documentation in compliance with state, federal, and organizational guidelines.
• Participate in student programs as appropriate, providing positive learning environments and evaluations.
• Actively pursue professional development through continuing education and involvement in professional organizations.
• Adhere to discipline-specific State Practice Acts and maintain professional licensure in good standing.
• Understand and participate in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• BLS Certification required prior to first shift.
• Must hold a valid, current State license as a Physical Therapist.
• Minimum of 2 years of experience is required.

Schedule

This position operates on a Day 5x8.0 schedule.
