PRIMARY JOB RESPONSIBILITIES

  • Evaluates, interprets, plans, implements, and assesses patient care based on their age and medical condition.

JOB REQUIREMENTS

  • Education: Must be a graduate from an accredited nursing program, which can be an Associate Degree, Diploma, or Bachelor of Science in Nursing.
  • Licensure / Certifications: Must hold a current nursing license in the state of practice (NC/SC) and possess CPR certification.
  • Experience: A minimum of one year as a registered nurse and at least two years of clinical experience is required. Additional experience may include roles such as LPN, CNA, military medic, EMT, or similar. Home health experience is preferred. For those with less than one year as an RN, one year of clinical experience as an LPN is necessary (internal use only). Therapy Assistants (PTA, OTA) with one year of home health experience and at least six months of RN experience may also be considered (internal use only).
  • Essential Technical / Motor Skills: Requires hand-eye coordination for administering injections, operating a computer, etc. Must be proficient in English communication and able to handle patient care equipment effectively.
  • Interpersonal Skills: Ability to establish positive interactions with patients, their families, physicians, and staff to ensure effective patient care.
  • Essential Physical Requirements: Must be capable of transferring or maneuvering objects weighing at least 50 pounds as part of patient care. Frequent lifting, moving, and positioning of patients is required, along with physical effort for bathing and ambulating patients. Occasional overhead reaching, stair climbing, and fine motor skills are also necessary.
  • Essential Mental Abilities: Must be able to assess patient conditions, develop care plans, choose appropriate interventions, evaluate responses to treatment, and educate patients about their health and recovery. Requires strong cognitive skills, including short- and long-term memory, prioritization, and the ability to learn new patient care techniques.
  • Essential Sensory Requirements: Ability to visually assess patients and utilize sight for implementing and evaluating care plans (e.g., changing dressings, starting IVs). Must also use hearing to auscultate lung and bowel sounds, respond to alarms, and communicate effectively with patients, families, and staff.
  • Exposure to Hazards: Potential exposure to noise, bloodborne pathogens, body fluids, infectious diseases, and needle injuries. May encounter traffic hazards and dangerous animals during home visits, as well as situations that could pose a threat to personal safety. Weather extremes and temperature changes may also be experienced.
  • Work Hours: Variable schedule from Monday to Friday, with potential weekend and holiday shifts as needed. Flexibility is required to meet staffing demands.
  • Population Served: Care for adolescents, adults, geriatrics, and pediatrics.
  • Must possess a valid driver's license from North Carolina and have access to a reliable vehicle.
