We are looking for a Primary Therapist who will work closely with clients to determine their rehabilitation needs and to provide individualized treatment by providing structured as well as holistic healing. Supports clients with foundational well-being that develops the skills needed to overcome, heal & grow to sustain long-term sobriety.
The Therapist is responsible for performing a wide variety of patient care activities as directed by the Clinical Director. They will be responsible for patient caseload and will provide group, individual, and family therapy. The Therapist will conduct psychosocial and biopsychosocial assessments. The Primary Therapist has primary responsibility for maintaining communication with the family, referral source and other service providers as needed.
JOB SUMMARY: HYBRID (SUD & MENTAL HEALTH)
The Primary Therapist serves as a key member of the clinical team, providing comprehensive therapeutic services to individuals experiencing substance use and co-occurring mental health disorders. Reporting to the Clinical Director (or Executive Director, as applicable), the therapist is responsible for managing an assigned caseload and delivering high-quality individual, group, and family therapy within an integrated treatment model.
This role includes conducting psychosocial and biopsychosocial assessments, developing and implementing individualized treatment plans, and providing ongoing clinical support, crisis intervention, and care coordination. The Primary Therapist maintains primary communication with families, referral sources, and external service providers as needed to support continuity of care.
As a cornerstone of both the substance use and mental health programs, the Primary Therapist fosters healing, resilience, and personal growth by delivering trauma-informed, evidence-based care. Through collaboration, clinical excellence, and holistic support, this role plays a vital part in helping clients achieve stability, recovery, and improved overall well-being.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Clinical Care & Caseload Management
Maintain and manage an assigned caseload of clients receiving substance use and/or mental health services, ensuring individualized, clinically appropriate care.
Provide evidence-based individual, group, and family therapy within an integrated SUD and mental health treatment model.
Facilitate family sessions and maintain ongoing communication with families, referral sources, and collateral providers as clinically indicated.
Deliver clinical services in accordance with the scope of practice, licensure, and program level of care (SUD and mental health).
Assessment & Treatment Planning
Complete psychosocial and biopsychosocial assessments within required timeframes.
Develop, update, and review individualized treatment plans in collaboration with clients, ensuring client participation and required signatures.
Provide regular treatment plan updates and clinical status reviews to support continuity of care and utilization review requirements.
Crisis Intervention & Clinical Support
Respond promptly and effectively to client crises, including relapse, safety concerns, and acute mental health needs, utilizing approved protocols and clinical judgment.
Collaborate with interdisciplinary teams to ensure appropriate crisis management, stabilization, and follow-up care.
Documentation & Compliance
Complete all clinical documentation (assessments, progress notes, treatment plans, family contacts, and utilization review documentation) accurately and within required timelines (e.g., 48-hour documentation standards).
Ensure documentation meets organizational standards, Joint Commission requirements, and federal, state, and local regulations.
Maintain strict confidentiality of client information in compliance with HIPAA, 42 CFR Part 2, and all applicable privacy laws.
Care Coordination & Collaboration
Coordinate client care with internal departments including Clinical, Medical, Admissions, and Utilization Review to ensure continuity and alignment with treatment goals.
Participate actively in multidisciplinary treatment team meetings, case consultations, and staffing as scheduled.
Communicate changes in client status, progress, or deviations from treatment plans to appropriate team members in a timely manner.
Professional Standards & Development
Accept and utilize clinical supervision, addressing feedback and areas for growth as identified by supervisors.
Maintain required licensure, certifications, and completion of ongoing training and continuing education.
Demonstrate cultural competency and provide inclusive, trauma-informed, and culturally responsive care.
Organizational & Regulatory Responsibilities
Adhere to all organizational policies and procedures, including those specific to Substance Use Disorder and Mental Health programs.
Demonstrate commitment to the organization’s mission, vision, and values.
Maintain compliance with Joint Commission standards, licensing bodies, and accreditation requirements.
Perform other duties as assigned to support program operations and client care.
Maintains compliance with Ongoing Training, HIPAA regulations, Joint Commission policies, as well as state-specific requirements

QUALIFICATIONS:
Master’s Degree in social work, psychology or related human services field required.
A minimum of 1 year of mental health counseling experience is required.
Clinical Licensure Requirements based on role location:
Mental Health
Tennessee - LCSW, LPC-MHSP, LMFT
SUD
Tennessee - LADAC I, LADAC II, LCSW, LPC-MHSP, LMFT
CPR Certified preferred
PHYSICAL DEMANDS:
Phone Use: Requires significant use of a cellular phone, necessitating fine motor skills, hand-eye coordination, and the ability to view screens for extended periods.
Sedentary Work: Primarily involves sitting most of the time, but may involve walking or standing for brief periods.
Computer Use: Requires significant use of a computer and office equipment, necessitating fine motor skills, hand-eye coordination, and the ability to view computer monitors for extended periods.
Lifting Requirements: Occasionally required to lift and/or move items weighing up to 20 pounds.
Repetitive Motion: May involve repetitive motions of the wrists, hands, and/or fingers due to extensive writing and typing.
Vision Requirements: Must be able to read documents and view information on computer screens; may also require the ability to adjust focus.
Hearing and Speaking: Must be able to communicate effectively with coworkers and clients, both in person and via electronic communication tools.
Vehicle Use: Requires focus and vision requirements for safe driving of clients
WORK ENVIRONMENT:
Clinical Setting: Work is primarily conducted in a clinical outpatient setting designed for therapy sessions, group meetings, and client consultations.
Interaction with Clients: Frequent direct interaction with clients who are dealing with substance abuse and related behavioral health issues. This includes individual and group therapy sessions.
Emotional Demands: The environment can be emotionally demanding due to regular exposure to clients dealing with complex and often severe emotional, behavioral, and psychological issues.
Confidentiality: A high level of discretion and adherence to all confidentiality requirements and regulations is required, respecting the privacy of clients at all times.
Multi-disciplinary Team Collaboration: Requires close and collaborative work with other health care professionals, including psychiatrists, nurses, social workers, and administrative staff.
Schedule Flexibility: May require flexible hours, including evenings and weekends, to accommodate the scheduling needs of clients.
Crisis Situations: Must be prepared to handle acute behavioral health crises professionally and ethically, sometimes with support from other team members.
Medical Testing Environment: Works in an environment where blood and urine samples are collected for drug screening purposes. Although not directly involved in the collection process, must be comfortable working in close proximity to medical testing procedures. The risk associated with these activities is very low, managed through strict adherence to health and safety protocols.
