Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience and OASIS proficiency for a 13-week contract based in the Salinas/Santa Cruz, CA area.

Details

• Home health setting, beginning in the Salinas area with potential reassignment to Santa Cruz based on patient census
• Day shift schedule, 8 hours per day (7:00 AM – 3:00 PM), 5 days per week
• Caseload of up to 5–6 patients per day with a standard productivity expectation of 30 points
• 1:1 patient ratio with an average of up to 6 visits per day
• Weekends may be required as needed
• No on-call requirements
• EMR: Home Health MD
• Mileage reimbursement provided at the IRS standard rate

Requirements

• Minimum 1 year of home health PT experience required
• OASIS experience required
• Current CA PT license required if applicable
• Current BLS certification required
• Must have reliable personal transportation
• Ability to travel up to 50+ miles for patient visits on occasion

Additional Information

• Scheduling system groups patient visits to minimize drive time and windshield time
• Documentation tools will be provided by the facility and must be returned at the end of the assignment
• Timely communication and documentation are expected throughout the assignment
• Business casual attire or scrubs are both acceptable
• No parking costs at the office location
• First-time travelers will not be considered for this position
• Local candidates are accepted
• 2 professional references from a supervisor within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration
• All time-off or scheduling requests must be submitted at the time of initial consideration
