Key Responsibilities Conduct comprehensive physical therapy evaluations for patients of diverse backgrounds and age groups, assessing strength, mobility, and functional capabilities. Design and implement individualized treatment plans that align with each client’s goals, ensuring they are evidence-based and tailored to maximize therapeutic impact. Regularly assess patient progress and modify treatment plans as necessary, adapting strategies based on the clinical responses and feedback. Document all patient interactions meticulously using our electronic health record (EHR) system, maintaining compliance with industry standards for SOAP notes and progress summaries. Educate clients and their families about recovery processes, therapeutic techniques, and home exercise programs, enabling them to take an active role in their rehabilitation. Collaborate with other healthcare professionals within the organization, leveraging multidisciplinary insights to enhance patient care for those receiving multiple services.
