In this role, you will coordinate and manage patient care in the home setting, helping individuals achieve their health goals while maintaining independence and quality of life. What You'll Do: Coordinate and manage patient care plans to ensure high-quality, patient-centered outcomes. Conduct comprehensive nursing assessments in the home setting. Collaborate with patients, families, physicians, and other healthcare professionals to develop and implement individualized care plans. Monitor patient progress and modify care plans as needed. Provide skilled nursing services as ordered by the physician. Administer medications, treatments, immunizations, and other prescribed interventions. Educate patients and caregivers regarding disease management, medications, treatments, and available resources. Advocate for patients and help navigate healthcare services. Maintain accurate and timely electronic medical record documentation. Supervise and collaborate with home health aides and other members of the care team. Ensure patient needs are continually assessed and addressed to promote optimal outcomes. What You'll Bring: The ideal candidate will be self-motivated, compassionate, organized, and detail-oriented. Basic computer proficiency, including electronic medical record documentation and patient care coordination.
