We are seeking an experienced Home Health Physical Therapist (PT) for a 13-week contract assignment providing in-home therapy services to adult and geriatric patients. This role is ideal for therapists with strong home health, orthopedic, neurological, and rehabilitation experience who are passionate about helping patients regain mobility and independence.

Responsibilities:

• Provide physical therapy evaluations, reassessments, and treatment plans in home health settings
• Develop and implement individualized therapy programs focused on mobility, strength, balance, and safety
• Assist patients recovering from orthopedic surgeries, stroke, neurological conditions, and chronic illnesses
• Educate patients and families on home exercise programs, safety, and injury prevention
• Complete OASIS documentation and maintain accurate patient records
• Utilize assistive devices and adaptive equipment during patient care
• Collaborate with interdisciplinary healthcare teams to support patient recovery goals
• Travel between patient homes within assigned service areas

Requirements: Minimum 1 year of Home Health PT experience required OASIS documentation experience required Active PT license required BLS certification required Strong experience with geriatrics, orthopedics, neurology, gait training, and post-surgical rehabilitation Excellent communication, documentation, and time management skills HomeCare HomeBase or MatrixCare experience preferred
