**Case Manager for DREAM Work Training Program**
Are you ready to join a dynamic team and make a difference? We seek a passionate and engaging Case Manager for our DREAM Work Training Program! In this role, you will be responsible for facilitating and implementing individualized, outcome-based planning processes for each participant in our program. You will also work closely with the Lead Case Manager to ensure that outcomes are implemented effectively and that data collection is accurate.
**Key Responsibilities:**
- Advocate for individuals to promote high standards of service.
- Provide daily support to program participants.
- Develop personalized programs that encourage vocational growth and enrichment.
- Train staff and ensure the proper implementation of individualized programs.
- Deliver excellent customer service to all stakeholders.
- Complete all 245D paperwork as required by licensing standards.
**Compensation and Schedule:**
- Pay: $23/hour
- Monday through Friday, 7:30 AM - 4:00 PM (no weekends!)

**Qualifications:**
- 1-3 years of experience in a related field .
- Capable of physical movement and lifting to 50 lbs.
- GED or high school diploma required.
- A valid Minnesota driver’s license is required.
- Must pass background study, OIG, and MVR clearance.
- CPR/First Aid certification
$35 - $45 per hour
