Position Snapshot: Independent Living Skills Training (ILST) services are designed to improve or maintain the ability of the waiver participant to live as independently as possible in the community. ILST assists in recovering skills that have decreased as a result of the onset of a disability. ILST will primarily target individuals with chronic or progressive illnesses to maintain essential skills.
Essential Functions:
- Conduct comprehensive functional assessments to identify strengths and weaknesses.
- Provide assessment, training, and supervision to an individual with issues related to self-care, medication management, task completion, communication skills, interpersonal skills, socialization, sensory/motor skills, mobility, community transportation, maladaptive behaviors, problem-solving skills, money management, pre-vocational skills, and skills to maintain a household.
- Provide ILST services as established NHTD and TBI program manual policy and procedures. *Manual is provided at the time of hire.
- Establish a professional relationship with participants and act as an advocate for them to ensure they are receiving appropriate, participant-directed services from providers.
- Attend provider meetings as directed.
- Performs ongoing recruitment via developing relationships to "market" AIM Independent Living Center as an ILST provider.
Education and Experience:
- Be an individual with a Bachelor’s degree with three (3) years of experience completing functional-based assessments of ADL and IADLs, developing comprehensive treatment plans, and teaching individuals with disabilities and/or seniors to be more functionally independent or
- Be an individual with an Associate’s degree with five (5) years of experience completing functional-based assessments of ADL and IADLs, developing comprehensive treatment plans, and teaching individuals with disabilities and/or seniors to be more functionally independent or
- Licensed Physical Therapist, Licensed Speech Pathologist, Registered Occupational Therapist, Registered Professional Nurse, Certified Teacher of students with disabilities, Certified Rehabilitation Counselor – Certified by the Commission on Rehabilitation Counselor Certification, and
- One (1) year of experience completing functional-based assessments of ADL’s/IADLs, developing a comprehensive treatment plan, and teaching individuals with disabilities, seniors, and/or Traumatic Brain Injury to be more functionally independent.
Additional Qualifications:
- Schedule flexibility to include some evenings and the ability to manage a complex workload in various environments.
- Strong interpersonal, communication, organization, writing, and computer skills.
- Ability to effectively interact and work with individuals from diverse backgrounds.
- Ability to multitask and work effectively in both a team and individual setting.
- Bilingual proficiency preferred.
- Valid driver’s license with satisfactory driving record and reliable transportation.
- Ability to bend, stoop, reach, push, pull, stand, walk, carry, kneel, and lift up to 50 pounds.
- Ability to carry out essential functions of the job without posing a risk or harm to the health and safety of self or others.
