Conducts regular, but at a minimum two home visits each program year to all assigned families or if necessary coordinates and schedules these visits at the Center or appropriate alternative location at the convenience of the family.Actively participates in scheduled case conferences and team meetings to discuss families, transportation, attendance, , health and social services related concerns, always maintaining an appropriate demeanor regarding families and coworkers and effectively resolves issues when possible including appropriately advocating on behalf of families with community based agencies and institutions incorporating SCAP’s guidelines and agency values.RequirementsBachelor's Degree in related field or Associates Degree in Human Services and two (2) years directly related experience.Family Development Credential preferred.Computer literacy required and the ability to consistently upgrade skills to remain up-to-date on the use of technology in the workplace.Knowledge and understanding of issues related to individuals and families in poverty.
