Experience Requirements

• Minimum 1 year of LPN/LVN experience
• Long Term Care (LTC) experience preferred
• Experience administering medications and treatments

License & Certification Requirements

• Active Colorado LPN/LVN License
• BLS/BCLS Certification

Job Summary

The Licensed Practical Nurse (LPN) provides direct and indirect nursing care to residents in a Long Term Care setting. The LPN is responsible for medication administration, treatments, resident assessments, documentation, communication with physicians and families, and supervision of CNA staff.

Key Responsibilities

• Provide direct resident care during assigned shifts
• Administer medications and treatments as ordered
• Monitor resident conditions and report changes appropriately
• Complete nursing documentation accurately and timely
• Document acute illnesses, physician visits, passes, and returns
• Communicate updates, changes, and new orders to MDPOA as required
• Supervise and direct CNAs providing resident care
• Promote teamwork across departments and shifts
• Participate in meetings and educational programs
• Maintain regular attendance and punctuality
