Sunnyvale, CA (Territory includes Atherton, Cupertino, East Palo Alto, Los Altos, Los Altos Hills, Menlo Park, Mountain View, Palo Alto, Portola Valley, Santa Clara, Stanford, Woodside, Sunnyvale, and neighboring cities as needed)

Key Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs with the primary goal of restoring function and preventing or minimizing disability following disease or injury
• Conduct initial admission and evaluation visits, and notify the assigned Clinical Team Manager of OASIS scores within established timelines
• Document observations, clinical findings, problems, skilled interventions and treatments, goals, and discharge plans
• Help clients reach their maximum performance and functional level within the limits of their capabilities
• Supervise Physical Therapy Assistants as applicable
• Perform other duties consistent with skills, training, and the goals of the position

Required Qualifications

• Active Physical Therapist license valid in the state of California
• Minimum of one year of Home Health experience
• Proficiency in evaluating and treating patients under medically prescribed physical therapy programs
• Ability to complete and submit OASIS documentation within established timelines

Preferred Experience / Skills

• Experience supervising Physical Therapy Assistants in a home health setting
• Familiarity with home health documentation systems and care coordination workflows
