Psychiatric Registered Nurse

Mohawk Opportunities is seeking 2 full‑time Psychiatric Registered Nurses. One full‑time nurse will serve as a clinical staff member of the Assertive Community Treatment (ACT) Team and the other full‑time nurse will serve as a clinical staff member of the Forensic Assertive Community Treatment (FACT) Team. Both teams are a community‑based multidisciplinary treatment team located in Schenectady, NY. Service provision is designed to promote recovery by improving psychiatric symptoms, preventing relapses, teaching skills, and providing direct assistance and securing necessary community resources. These services may occur in the home, community, or office. Responsibilities Conduct person‑centered comprehensive health assessments. Develop and implement strength‑based treatment plans that are responsive to client needs and facilitate achievement of goals as needed. Oversee all aspects of medication management in compliance with agency procedures and protocols. This includes administration of injectable medications, managing Clozaril protocols, ordering medication, overseeing processes for medication distribution, utilizing a medication step system toward client self‑management, and managing all aspects of the medication room. Develop and maintain trusting and supportive relationships with clients, families, and community providers. All service provisions shall be judgment‑free, non‑confrontational, yet honest, open, and delivered in a professional, caring, supportive, and growth‑producing manner. Provide support, treatment, case management, community advocacy, and linkage services per the treatment plan. Complete required documentation of interventions as well as clients’ responses and outcomes. Readily respond to all crisis situations and urgent needs using sound clinical judgement to ensure the safety and well‑being of the individual. Provide on‑call crisis intervention services on a rotating schedule among team members, act to ensure client safety, assess needs, provide support, and intervene immediately as needed. Mohawk's programs and offices are multi‑level with stairs only. All employees must be able to travel to all levels of the facility in a safe and reasonable timeframe. Client services in the community include visits to clients’ apartments which can be on the second and third floors. Provide transportation to clients when necessary, using agency vehicles. In addition, an employee must have a valid NYS driver’s license and must be an approved driver, always driving safely, using all safety devices, and being medically able to drive. Employees must have access to and be willing to use their personal vehicle for business purposes. Qualifications Licensed Registered Professional Nurse by New York State. Bachelor’s degree in nursing preferred. Three years’ experience in a psychiatric setting, with at least one year’s experience in an outpatient setting preferred. Strong commitment to the rights and ability of each person with a mental illness to make their own decisions and choices, live in the community, work, and access continuous support and services to enhance wellness. Ability to effectively work with all people and demonstrate skills and competence to establish supportive and trusting relationships with all individuals. Uphold individuals’ rights and personal preferences in treatment, treat everyone with respect and dignity, and act as a courteous visitor in personal homes and family homes, demonstrating active listening. Valid and insurable NYS driver’s license. Must have own vehicle for business purposes; agency vehicles are available for client transportation. Salary, Schedule, and Benefits Salary $85,000/annual equivalent. Sign‑on bonus of $1,500. Monday through Friday 8:30 am–4:30 pm.
