Registered Nurse (RN) – Home HealthWe are seeking an experienced and compassionate Registered Nurse (RN) to join our growing Home Health team serving patients throughout Albuquerque and Rio Rancho.This position is ideal for an RN who has a strong background in home health, is comfortable working independently in the field, and has hands-on experience completing Start of Care (SOC) assessments and OASIS documentation.We offer a competitive salary for qualified candidates, along with the opportunity to join a growing home health team where your clinical experience and expertise are valued.Minimum QualificationsCandidates must meet the following experience requirements:Active New Mexico Registered Nurse (RN) license in good standingMinimum 2 years of total nursing experienceMinimum 1 year of Home Health nursing experienceMinimum 1 year of Start of Care (SOC) and OASIS experienceStrong clinical assessment and documentation skillsAbility to manage patient care independently in a home settingExcellent communication, organization, and time-management skillsReliable transportation and ability to travel throughout the Albuquerque/Rio Rancho service areaResponsibilitiesAs a Home Health RN, you will help patients receive high-quality clinical care while remaining safely and comfortably in their homes. Responsibilities may include:Complete comprehensive Start of Care (SOC) assessmentsComplete accurate and timely OASIS documentationPerform skilled nursing assessments and visitsDevelop and update individualized plans of careMonitor changes in patient condition and communicate with physicians and the interdisciplinary care teamProvide patient and family educationCoordinate care with therapists, physicians, caregivers, and other healthcare professionalsMaintain accurate, timely, and compliant clinical documentationHelp identify changes in condition and opportunities to improve patient outcomes and prevent unnecessary hospitalizationWe're Looking for an RN Who...You understand that great home health nursing requires more than strong clinical skills.
