As a Critical Access Hospital serving East Central Illinois and Northwest Indiana, IMH offers physicians the opportunity to practice broad-spectrum primary care while working closely with specialists, hospital providers, nursing homes, and community partners. Position Highlights Outpatient primary care practice in a Rural Health Clinic setting Opportunity to build long-term relationships with patients and families Collaborate closely with experienced Nurse Practitioners, specialists, hospital providers, and care management teams Participate in population health initiatives, including Annual Wellness Visits, chronic disease management, and preventive care programs Opportunity for nursing home rounding. Access to on-site laboratory, imaging, specialty clinics, rehabilitation, and hospital services Supportive leadership team committed to provider engagement and reducing administrative burden Competitive compensation and benefits package (Base salary package starts at $325K and includes s sign on bonus and relocation package.)
