Post Transplant Clinic RN - Scripps Green Hospital

This is a Full-Time position with a Monday-Friday schedule. No weekends or holidays are required. Position is located at Scripps Green Hospital. We invite you to join our Organ Transplant Clinic team as a Post Transplant Clinic Nurse. As a Post Transplant Clinic Nurse, you will formulate custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. In addition, you will be responsible for: Establishes specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance. Assists patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Identifies and aligns resources that impact high-risk care. Coordinates services on behalf of the high-risk patients. Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed. #J-18808-Ljbffr Scripps Health
