Responsibilities
- Perform initial evaluations and ongoing treatment for a home health/outpatient caseload
- Develop and implement individualized plans of care with measurable goals
- Reassess patient progress, adjust therapy plans, and document outcomes
- Educate patients and caregivers on exercises, home programs, safety, and prevention
- Coordinate with an interdisciplinary team; communicate progress and assist with discharge planning
- Maintain accurate documentation, ensure infection control, and uphold patient safety standards

Requirements
- Education: Bachelor's degree in Physical Therapy from a four-year accredited college or university (or degree in a related field)
- Licenses/Certifications: Current California Physical Therapy license, or eligible for registration; AHA BLS required
- Experience: 1–2 years related experience and/or training, or equivalent
- Certifications: AHA BLS required
- Additional: Valid California driver’s license when travel is required; (APTA membership is preferred)
