We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our evening shift team at Fort Wayne Health Campus. As a key member of our healthcare team, you will provide high-quality care to our residents, delivering exceptional service and support in a warm and welcoming environment. Responsibilities:

Provide skilled nursing care to residents, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans to meet the unique needs of each resident
Collaborate with interdisciplinary teams to ensure comprehensive care and support
Provide emotional support and comfort to residents and their families
Maintain accurate and detailed records of resident care and progress
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

Current LPN licensure in the state of Indiana
1+ year of experience in long-term care or a related setting
Ability to work effectively in a fast-paced environment with minimal supervision
Excellent communication and interpersonal skills
Compassion and empathy for residents and their families
