Travel LPN – Long-Term Care

Location: Falmouth, MA 02540
Duration: 13 weeks, with possibility of extension
Schedule: 40 hours per week
Shifts: Rotating Days, Evenings & Nights
Shift Times: 3:00 PM–11:30 PM and 11:00 PM–7:30 AM
Weekend/Holiday: Every other weekend and holiday rotation

Weekly Travel Pay: $1,850–$2,084, depending on experience

Position Overview

We are seeking an experienced Licensed Practical Nurse (LPN) to provide direct nursing care to residents in a long-term care environment. The LPN will also supervise nursing assistants and coordinate day-to-day nursing activities to ensure residents receive safe, compassionate, and high-quality care in accordance with applicable federal, state, and local regulations.

Key Responsibilities

• Provide direct nursing care to residents according to established care plans and applicable standards.
• Supervise and coordinate the daily activities of nursing assistants and support personnel.
• Participate in planning nursing services, programs, and activities for assigned shifts.
• Complete required documentation for resident admissions, transfers, and discharges.
• Review resident charts, treatment plans, medication orders, diets, and care requirements.
• Prepare and administer medications and treatments as ordered by physicians.
• Verify resident identity before administering medications or treatments.
• Receive and accurately document physician telephone orders.
• Transcribe physician orders to resident charts, medication records, treatment plans, and other required documents.
• Maintain timely, accurate, and informative nursing documentation reflecting care provided and resident responses.
• Notify nursing staff regarding new admissions, expected arrivals, room assignments, and other relevant information.
• Make rounds with physicians and consult with providers regarding resident care, treatment, and rehabilitation needs.
• Monitor residents for changes in condition and communicate significant findings to appropriate healthcare providers.
• Perform nursing procedures including catheterization, tube feedings, suctioning, dressing changes, colostomy and drainage care, specimen collection, and other treatments within the LPN scope of practice.
• Collect urine, blood, sputum, and other specimens and perform routine laboratory testing as required.
• Monitor and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Ensure adequate supplies, equipment, and floor-stock medications are available to meet resident care needs.
• Provide health education and instructions to residents and family members.
• Participate in resident and family orientation as appropriate.
• Support facility surveys and inspections conducted by authorized regulatory agencies.
• Maintain strict confidentiality of resident information and protected health information.
• Uphold resident rights and report suspected unauthorized disclosure or other concerns appropriately.
• Provide compassionate and respectful service to residents, families, visitors, and coworkers.
• Assist with care of residents who are critically ill, dying, or deceased as required.
• Perform other nursing and administrative duties as assigned.

Required Qualifications

• Graduate of an approved LPN program or equivalent nursing education from an accredited institution.
• Current, active, and unencumbered Massachusetts LPN license .
• Current BLS/CPR and AED certification .
• Previous long-term care or skilled nursing experience preferred.
• Ability to read, write, speak, and understand English.
• Knowledge of nursing practices, medical procedures, and regulations applicable to long-term care.
• Strong leadership and supervisory skills.
• Ability to make independent decisions and exercise sound clinical judgment.
• Strong communication and interpersonal skills.
• Ability to work effectively with residents, families, physicians, nursing assistants, and other healthcare professionals.
• Ability to organize, prioritize, and coordinate resident care activities.
• Commitment to maintaining resident dignity, privacy, safety, and confidentiality.

• Must be able to provide care appropriate to the age-related needs of residents.
• Must demonstrate patience, professionalism, compassion, and a positive attitude when working with residents and families.
• Must be adaptable and willing to incorporate updated nursing practices and procedures.
• Must be able to communicate relevant information regarding resident conditions.
• Must be physically capable of lifting at least 50 pounds and meeting the physical and sensory requirements of the position.
• Must be able to work rotating shifts, including every other weekend and holiday rotation .
