Compensation

Pay Range: $40.07 - $48.10 per hour, depending on prior experience.

What You Bring (Requirements)
  • Graduation from an accredited nursing program; a Bachelor's degree is preferred.
  • Current RN license in the state of Colorado.
  • A minimum of two years of nursing experience, including long-term care, dementia care, or acute care; home care experience is preferred.
  • CPR certification.
Your Responsibilities

The Registered Nurse (RN) will manage cases, coordinate care, supervise, and implement professional and supportive services for clients, utilizing nursing theory and processes in line with the Nurse Practice Act. Key responsibilities include:

  • Reviewing client referral information and responding to inquiries as needed.
  • Conducting client assessments according to agency standards to ensure effective home care services.
  • Independently directing, planning, and initiating appropriate actions in home care situations.
  • Performing duties in accordance with agency policies, procedures, and community standards.
  • Completing additional tasks as assigned by the Director of Clinical Services.

Our organization is a Medicare/Medicaid-certified home health care agency located in Montrose, CO. We have a dedicated team of nurses, rehabilitation therapists, and home health aides focused on delivering personalized healthcare with compassion and empathy. We specialize in caring for older adults, providing in-home health services across Delta, Montrose, and Ouray counties.

Experience
  • Required: 2 years of nursing experience, including long-term care dementia care or acute care.
  • Preferred: Experience in home care.
Education
  • Preferred: Bachelor's degree in Nursing or a related field.
Licenses & Certifications
  • Required: CPR/First Aid certification and Registered Nurse license.
