Participate in the development of patient treatment plans. Perform comprehensive bio-psychosocial assessments. Provide patient and family education, support, and advocacy. Collaborate with a multidisciplinary team to ensure continuity of care. Conduct Case Management and Utilization Review activities. Manage discharge planning and coordinate patient transitions of care. Provide weekend coverage for up to two units, with a focus on discharge planning. Required Experience Minimum 2 years of acute hospital case management experience required. Schedule & Coverage Requirements Weekend Requirement: Every other weekend. Must be available for holiday coverage . Expected to cover up to two units on weekends and prioritize discharge planning. Licensing Requirements California State License: Preferred.
