Job Title: Adult Nurse Case Manager Profession: RN Specialty: Case Management Duration: 14 Weeks Shift: Day Hours per Shift: 8:30 AM - 5:00 PM Experience: 2 years RN experience, current in-patient Case Management experience License: Active RN License Certifications: BLS Must-Have: EPIC and post-acute facility referral platforms experience Description: The Adult Nurse Case Manager coordinates care for a select caseload of patients. This includes triage, care coordination, and safe transfer to home. The role requires proactive communication with payors to ensure approval of treatment plans and required resources. The Case Manager maintains knowledge of and responds to clinical systems and fiscal data related to specific patient populations. Collaboration with the multi-disciplinary team, patients, and their families is essential to ensure effective and efficient transition planning. The Case Manager coordinates transitions to post-acute settings, including nursing facilities and acute inpatient rehab. Patients are managed based on departmental procedures, maintaining clinical safety and quality. Entering and maintaining patient information in all departmental systems is required per operational guidelines. Current and active CPR certification is necessary for this role.
