_Lakeport Post Acute Care is a skilled nursing and rehabilitation facility located in Lakeport, CA near beautiful Clear Lake and is approximately 125 miles northwest of Sacramento. This 81 bed facility is ideally looking for submitted candidates to have completed at least one 13-week assignment in a skilled nursing facility, however this is not a requirement._ Requires 3 references from 3 most recent employers to submit.
