Key Responsibilities

• Conduct comprehensive patient assessments to determine functional limitations and physiotherapy needs
• Develop and implement tailored care plans incorporating therapeutic exercises, manual therapy techniques such as trigger point therapy, and patient education
• Manage discharge planning processes to ensure seamless transitions from hospital to home or community settings
• Provide specialized care for diverse populations including pediatrics, geriatrics, orthopedics, and patients in acute care or outpatient clinics
• Utilize EMR (Electronic Medical Records) systems to document progress, treatment plans, and clinical notes accurately
• Collaborate with multidisciplinary teams to coordinate patient care and optimize recovery outcomes
• Perform home & community care visits to support patients' independence and promote ongoing wellness outside clinical settings

Required Skills

• Strong knowledge of physiology and human anatomy to inform effective treatment strategies
• Proficiency with medical terminology and familiarity with physiotherapy techniques including trigger point therapy and orthopedics
• Experience in discharge planning, care plan development, and patient education across various care settings
• Ability to work effectively with EMR systems for documentation and communication purposes
• Excellent communication skills for engaging with patients of all ages including pediatrics and geriatrics and collaborating with healthcare teams

Education Requirements

• Valid licensure as a licensed Physical Therapist in the applicable region or country (NH/MA) 3 locations (Plaistow NH / Exeter NH / Newburyport MA)

Required Experience

• Demonstrated experience in inpatient hospital settings, outpatient clinics, or home health environments
