Job Title Occupational Therapist Location Ashland, WI | Skilled Nursing Facility Schedule / Job Type Contract | Days | 13-Week Assignment Key Responsibilities
- Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
- Implement therapeutic interventions to improve patients' functional abilities and independence in daily activities
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care team members to ensure coordinated patient care
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
- Active Occupational Therapist (OT) license in the state of Wisconsin
- Graduate of an accredited Occupational Therapy program
- Ability to work day shift in a skilled nursing facility environment
- Prior experience working in a skilled nursing facility or long-term care setting
- Familiarity with Medicare documentation and compliance requirements
- Expected 35 hours per week providing a consistent and stable schedule throughout the contract
- 13-week contract offering a defined engagement with an opportunity to make a meaningful impact in a SNF setting
