Transitional Care Nurse - Discharge & Care Coordination

Saint Luke's Hospital of Kansas City is seeking a Transitional Care Nurse to coordinate safe, patient-centered transitions from hospital to home or post-acute settings. You will lead discharge planning, assess risks, reconcile medications, and provide clear education to patients and families to prevent avoidable readmissions.
