This role focuses on delivering patient-centered care in home or hospice settings, prioritizing dignity, comfort, and quality of life for patients and their families across the Midwest region.EducationGraduation from an accredited nursing program and current licensure as a Registered Nurse eligible to practice in the region are required.ExperiencePrevious experience in hospice, palliative, or critical care is preferred. Familiarity with hospice philosophy and training is essential.RequirementsCurrent Basic Life Support (BLS) certification by the American Heart Association standards.Completion of Mandatory Reporter training within six months of hire.Strong communication skills, both oral and written, to effectively interact with patients, families, and healthcare teams.Basic computer proficiency and ability to perform simple mathematical calculations.Adherence to all federal, state, and professional licensure and regulatory training requirements.Key Responsibilities:Conduct comprehensive assessments addressing physical, emotional, and spiritual needs of patients.Develop and implement individualized care plans in collaboration with interdisciplinary teams.Administer medications and manage symptoms following established protocols.Provide education and emotional support to patients and families regarding end-of-life care options.Coordinate services and resources to support patients and caregivers throughout the hospice journey.Maintain accurate documentation and communicate patient status changes promptly to the care team.Perform venipuncture or collect lab specimens as ordered.Ensure timely notification of pharmacies and providers regarding medications and supplies.This position offers a warm, empathetic environment where the individuality and preferences of each patient are respected and supported.
