Fellowship offers a comprehensive continuum of services that include Independent Living, Personal Care, Skilled Nursing, Short-Term Rehabilitation and Memory Support — all in our beautiful senior living community.Your MissionAs a PT Social Worker, you’ll partner closely with the SW/CM to drive strong resident outcomes by supporting each person’s physical, emotional, and psychosocial well-being. This role exists to keep transitions smooth, communication clear, and resident rights front and center—every single day.Key responsibilities include assisting with discharge planning, coordinating communication between residents, families/representatives, and the interdisciplinary team, and advocating for resident rights in line with Fellowship standards.Required qualifications/skills: - Bachelor’s degree in Social Work- Minimum two years’ experience in geriatric care- Excellent interpersonal/public relations skills; strong organization- Ability to counsel individuals in crisis; knowledge of senior community resources- Intermediate computer skills; working knowledge of the MDS- Understanding of Medicare/MA/managed care/private pay basics; medical terminologyIf you’re ready to bring steady compassion and high-energy professionalism to Whitehall, apply today.A Shift That MattersYou step onto our onsite Whitehall campus and quickly get your bearings—checking updates, priorities, and who needs an extra touchpoint today.
