Job Description: Summary: Working with IDT (Interdisciplinary team) to develop a plan of care. Implementing the plan of care. Obtaining physician's orders when necessary. Providing those services requiring substantial and specialized nursing skill. Educating the patient and family in the care of the patient, including pain management. Evaluating outcomes of services. Coordinating and communicating the patient's physical or medical condition to the IDT. Preparing clinical notes, and supervising licensed practical nurses and home health aides/attendants providing delegated nursing services. Coordinating the implementation of each patient's plan of care. This position may be required to cross train within the agency. Responsibilities in secondary position may require access to medical records and other computer access exceeding routine computer usage.

Requirements: Associate's or Bachelor's degree in Nursing from an accredited school. Must have minimum two years experience. Med/Surg, critical care, adult/geriatric nursing preferred

Licenses and Certifications: Valid and active RN licensure from appropriate state CPR required Must have current driver's license and carry liability/collision insurance
